	Copies of the last 3 year end financial statements for the company (or tax returns if statements are not available)
	Current interim statement for the company (balance sheet & income statement)
	Copy of the most current personal financial statement on each owner	(148KB)
	Contractor's questionnaire	(128KB)
	Resumes for each owner
	Current work in process schedule	(32KB)
	Bank Authorization
	Current Certificate of Insurance for the company
